Submitting for a Course Approval (CEU Locker)
Submit your course approval application through CEU Locker's Provider Page.
CEU Locker provides a fast, convenient centralized location to get approvals in several APTA Chapters by completing and storing your paperwork requirements only one time.

HOW TO SUBMIT A COURSE:
Set up a Provider account.
On your Provider Dashboard, use the large yellow button to begin a course application entry. Enter all details you have; you can save a draft if you need to return at a later time.
Once you have all of your information, and course dates/locations, you will use the SUBMIT icon on the dashboard listing entry, inline with your course title.
The first step in submitting a course application is to choose the state association you'd like to submit to.
The state choice will take you to the specifics on pricing and requirements page, allowing you to make choices there.
Once you are complete with those choices, you'll submit payment and the course application will be submitted.
You should receive an email notice of submission.
At this point, the course application will land back on your dashboard listing, and be LOCKED from further changes.

- References should be current (within past 5 years) and PEER REVIEWED
- Please include course schedule and break times for accurate CEHs granted.
- One course title and hours earned per application: Courses offered in a series must be submitted separately (attendees register for courses separately).
- Courses offered congruently on the same day and sponsored by one company / location (i.e. – symposiums, Annual Conference) must be submitted separately.
- The review will not begin until payment is received.
- Applications will not be accepted if the course has already occurred.
- Application review will take approximately 3 weeks. Incomplete applications can be delayed.
- Once approval is granted, you will receive the letter electronically. This course approval is good for one year from approval date. (See below for renewal process)
